In-Network

Kaiser, HMA, Regence, Regence BCBS, Anthem, Medicare, Kaiser or Regence Medadvantage plans, First Choice Health Network, Cigna, Labor & Industries, and PIP/auto plans currently accepted.

Telehealth coverage varies for each insurance and each individual’s plan, so please contact your insurance to confirm your online telehealth PT coverage. Due to COVID-19 recommendations and positive effects of teleheath post-pandemic, many insurance companies are covering telehealth PT.

Out-of-Network

Momenta is considered Out-of-Network (OON) with all other insurances. Check your policy by calling or looking at your online portal for Out-of-Network coverage details. We currently offer Superbill receipts if needed that you can submit to your insurance after self-pay for full or partial reimbursement depending on your policy. Our services are eligible for HSA/FSA payments.

We only bill Premera and Aetna plans with OON coverage on your behalf.

Unfortunately, due to Medicare laws, we are not able to see any OON Medicare Advantage patients as self-pay. As a contracted traditional Medicare provider, we are legally not allowed to accept self-pay from patients with these policies.

Please contact your insurance provider to understand your physical therapy benefits, to confirm if you are in-network, and ask if a referral for PT services is required. If authorization is required we will handle this on your behalf!
