In-Network Insurance Plans

Kaiser, HMA, Regence, Regence BCBS, Premera, Lifewise, Anthem, Cigna, Medicare, Kaiser-Medicare Advantage, Regence-Medicare Advantage, or Premera-Medicare Advantage, First Choice Health Network, Labor & Industries, and PIP/auto plans currently accepted.

Telehealth and dry needling coverage varies for each insurance and each individual’s plan, so please contact your insurance to confirm your online telehealth and dry needling PT coverage.

Out-of-Network Insurance Information

Momenta is out-of-network (OON) with all other insurance plans. If your plan is out-of-network with our clinic, we recommend checking your policy’s out-of-network benefits by contacting your insurance carrier or reviewing your online member portal, as some of your visits may be reimbursed to you.

For commercial OON plans, services are provided on a self-pay basis. We are happy to provide a Superbill upon request, which you may submit to your insurance for possible partial or full reimbursement, depending on your plan. Our services are eligible for HSA/FSA payment.

In 2026, we can not bill out-of-network plans on your behalf.
